SUBJECT: FORMAL COMPLAINT, BILLING DISPUTE, AND REQUEST FOR MEDICAL RECORD AMENDMENT To the Clinic Manager at urgent care 201 n mayfair rd southeastern wisconsin: I am disputing a $1,000 bill and filing a formal complaint against the urgent care nurse (older white lady with blonde hair). During my visit, I told her my injury involved third-party liability. I tried to give her the insurance details, but she refused to write them down. Because she ignored this info, I was wrongly billed $1,000. She also refused to document my actual medical symptoms, including oxygen intake issues. When I insisted on accurate documentation, she falsely charted that I was "hostile" and "disruptive." I was not hostile; I was an injured patient trying to get proper care. I demand the following actions: Freeze the Bill: Put the $1,000 charge on hold so it does not go to collections. Fix the Billing: Remove the charge or bill the correct third-party insurance. Fix My Chart: Under HIPAA law, correct my medical record by removing her false behavior claims and adding my real symptoms. Investigate: Review this nurse's unprofessional conduct and refusal to listen .Please reply in writing within 7 days to confirm these corrections. Sincerely disrespected injured patient

This has to do with Labcorp inside the Ascension Mayfair Wauwatosa location. They decided to "update" their system after a holiday weekend and it has been a disaster! You no longer check in at the front counter but go to the express kiosk in the waiting room and express is only in the name only. I checked in at 11:15a.m. and it is currently 12:30p.m. and I am still waiting to get my blood draw. Where the old system took you minutes to get in and out, the new express system takes over an hour plus to get your blood drawn! And there are only 2 techs in the lab, which with rolling out a new system, here is an idea, have extra techs on hand to speed things up. And the techs have every excuse under the sun why it is taking so long instead of just moving people along.

If I could give this place o stars I would. I came Friday the 27th. I wasn't even seen by a doctor but by a nurse practitioner who was horrible. She prescribed me prednisone.I can't take prednisone because iam allergic to. She also sent my prescription to the wrong pharmacy. She said she. would change the pharmacy. I didn't get any medication and iam still sick and suffering. So people please do not come to this place for treatment. I wouldn't recommend this place to my dog this place is so expensive and they don't do nothing for you. I will dispute my bill and iam never coming back. I was sick all day at work and to come here and not be treated. that is sad.

I went in yesterday at 9:18pm and the receptionist said they were closed, I asked how so if online it clearly stated the clinic closed at 10pm and the nurse or whatever she was so “kindly” told me that their system was down and they were over capacity so they were no longer taking in any other patients, which was weird because there was not a lot of people in the lobby, but what do I know. So get it, it is all about the money for you all and you don’t care about people or their well being, but at least update your hours online if you are going to close the clinic earlier, because turning someone down when they are sick is not okay.

Avoid this location, went in with all the signs and symptoms of strep. Waited 45 minutes to get tested and it came back negative. Went to my regular doctor the next day when symptoms worsened and had a positive strep result.
